Levy PME/Forklift and MEWP Operator Pre-work:

Before your in-person training with CDF Training LLC at Levy, you must complete this pre-work.

Please complete the steps below BEFORE the scheduled class date:

1. Watch the PME/Forklift training video.
2. Download and complete the PME/Forklift workbook.
3. Complete the sign in form documentation.

This pre-work is required before you can complete the hands-on practical evaluation and course sign-off with certificate issuance.

Complete all sections before the in-person class. Your instructor may review your answers and will use this workbook, along with the practical evaluation, to verify training has been completed.
